How to choose a pillow: the complete guide
You may already have changed pillows two or three times without anything improving. It is the most common experience there is, and it has a simple explanation: almost everyone chooses on the wrong criterion.
We compare fillings. Foam, feather, latex, microfibre. We read the descriptions, we compare prices. And we miss the one parameter that actually decides.
Height before material
A study published in Applied Ergonomics compared the influence of material, firmness, shape and height on cervical alignment during the night. One factor came out clearly ahead of the others: height.
In other words: a premium memory foam pillow at the wrong height will do you more harm than a basic pillow at the right one.
The reason is mechanical. Your head weighs between four and five kilos. If the pillow does not support it at exactly the right height, the neck muscles work to hold it — seven to eight hours a night, every night. That prolonged contraction is what you feel on waking.
The ten-second test
Before we talk numbers, do this. Lie down in your usual position and ask someone to look at you from the front, or take a photograph.
Your ear should line up with your shoulder. Your spine should form a continuous line, with no break at the neck.
If your head tips upwards, the pillow is too thick. If it leans towards the mattress, it is too flat. That is all, and it settles most of the question.
This test costs nothing and beats any specification sheet. Do it with your current pillow before buying another: you will know immediately whether it is at fault.
The height you need, by position
You sleep on your side
It is the most common position, and the one that needs the most height: 10 to 15 cm.
The reason is purely geometric. On your side, your shoulder creates a gap between your ear and the mattress, and the width of that gap matches the width of your shoulders. The pillow has to fill it completely — otherwise your head drops all night.
It is also why broad-shouldered sleepers need a distinctly thicker pillow than average, and why "universal" pillows almost always disappoint them.
You sleep on your back
You need less height: 8 to 12 cm.
On your back there is no shoulder to compensate for. The goal changes: it is about supporting the natural curve of the neck without pushing the head forward. Too thick a pillow brings the chin towards the chest, which stretches the muscles at the back of the neck all night and can also restrict breathing.
You sleep on your front
Let us be direct: this is the hardest position to reconcile with a relaxed neck. Sleeping face down forces you to turn your head 25 to 35 degrees simply to breathe, and to hold that rotation all night.
If this is your position, the rule inverts: you need very little height, under 7 cm — or no pillow at all under your head. Many physiotherapists suggest sliding a thin one under the hips instead, to stop the lower back from arching.
Measure your own
The ranges above are averages. To refine them, there is a simple benchmark if you sleep on your side.
Standing against a wall, measure the distance between the wall and the hollow of your neck, at shoulder level. That distance, to within a centimetre, is the pillow height you need once it is compressed under the weight of your head — not the height printed on the packaging.
That distinction is decisive, and we come to it now.
The collapse trap
A conventional pillow compresses. Its height when you lie down is not the height it will have at three in the morning.
So you chose one height and you are sleeping on another. It is the most common flaw of fibre and feather fills, and it is invisible at the point of purchase: in the shop, the pillow is new and plumped.
This is what explains the widespread feeling that "the pillow does not hold". It does not hold, indeed — it holds for three weeks.
Fillings, without flattery
Memory foam keeps its height and spreads pressure instead of concentrating it. That is its real advantage, and it matters: a correctly chosen height stays correct until morning. Its known flaw is heat: dense foam retains body warmth. Good design compensates with ventilated side panels or an open-cell structure; poor design does not compensate at all.
Feather and down are pleasant and adjustable, but collapse during the night and need regular plumping. Height is inherently unstable.
Latex holds well, breathes better than foam, and lasts. It is firmer and more expensive, and its bounce does not suit everyone.
Microfibre is cheap and washable. It flattens quickly, and should be treated as a consumable to replace every year.
Density, and what it means
For memory foam, density is expressed in D — 40D means 40 kg per cubic metre.
Below 30D, foam flattens permanently within months. Around 40 to 50D, it holds for years. Above 60D it becomes very firm and the benefit is debatable.
A seller who cannot tell you the density of their foam does not know it — and there is a reason for that.
Classic or contoured shape
A contoured pillow, often called ergonomic, has a raised ridge under the neck and a hollow centre for the back of the skull. On your back, that design has real logic: it supports the neck curve instead of pushing the head.
On your side, its value depends entirely on the height of that ridge. If it matches your build, it works better than a flat pillow. If it does not, the shape changes nothing.
Models offering two heights depending on which way up they are used solve part of the problem: they let you adjust without buying again.
When to replace it
A simple test: fold your pillow in half and let go. If it does not spring back immediately, it is finished.
Allow one to two years for microfibre, two to three for feather, three to five for dense foam or latex. Beyond that, what is under your head bears no resemblance to what you bought.
The four most common mistakes
Choosing on material. Height decides. Material mainly determines how long that height will be respected.
Keeping the same pillow after changing position. If you have moved from back to side, your requirement has changed by several centimetres.
Judging after one night. An ergonomic pillow changes a position your body has held for years. Allow three to ten nights before concluding. The first ones can be uncomfortable without the product being at fault.
Ignoring heat. The head releases a meaningful share of body heat, and the pillow is the longest point of contact of the night. A pillow that has you hunting for the cool side wakes you as much as one that is badly sized.
What a pillow does not do
Everything above describes the mechanics of position. A pillow is a comfort product: it is not a medical device, it treats no condition and replaces no professional advice.
It acts on one variable — alignment over seven to eight hours. That is a real, daily variable, and one of the few you can change tonight. It is not the only one.
See a doctor or a physiotherapist if your pain is severe, persists beyond a few weeks, or comes with pins and needles, numbness or loss of strength. No bedding accessory belongs in that conversation.

On your side, aim for 10 to 15 cm. On your back, 8 to 12 cm. On your front, under 7 cm or nothing. Check the ear-shoulder alignment — it is the only test that counts. And choose a filling capable of holding that height until morning.
